      Immunization

      To enroll your child for school, a signed Certificate of Immunization (CIS) downloaded from the Washington State Department of Health Immunization Registry (WAIIS) is required. To print your child's immunization certificate from the WA State Immunization Registry, go to wa.myir.net, or contact your healthcare provider. 

      VISD no longer uses Skyward for immunization management. Rather, the District uses the Department of Health's Immunization Registry. In order for us to to keep your student's records up to date, we need permission to enter vaccine doses into WAIIS. Please complete and sign this FERPA form and email to nurses@vashonsd.org.

      If a CIS isn't available online, please sign a blank CIS form (located here) and attach copies of medically verified immunizations from your healthcare provider.     

      If the parent/guardian chooses not to immunize for any or all of the required immunizations, a Certificate of Exemption (signed by parent and Health Care Provider) is required.

      Please contact the school nurse for further assistance if you are unable to obtain immunization records.

      • VISD Head Lice Procedure:
        If live head lice are found on a child’s head during the school day, the family will be notified and the child will remain at school for the remainder of the day.  The child can return to school once the head lice have been treated and there are no remaining live lice on the head.  The child should check in with the school nurse upon arrival back to school. After each treatment, checking the hair and combing with a nit comb to remove nits and lice every 2-3 days may decrease the chance of self-reinfestation. Continue to check for 2–3 weeks to be sure all lice and nits are gone, per the CDC 

      Life-threatening Illness

      Per Washington State Law, Health Care Provider orders, medications and/or supplies must be in place no later than the first day of school. Parents must meet with the district nurse to establish a plan of care before school begins (preferably two weeks in advance).
